Required Reports for Guardians

Annual Status Report for an Adult 

This reporting form provides the Court with information about the person who has been appointed a guardian under the Treatment and Care of the Developmentally Disabled Act or Title 15, Chapter 5 of the Probate Code. Do not use this form if you are the guardian of a child.

 

Annual Status Report for a Minor

This form provides the Court with information about a child who has been appointed a guardian. This form should NOT be completed for an adult who has a guardian or an individual with a developmental disability.  

 

 

Required Reports for Conservators

90 Day Inventory

The purpose of this report is to give the court as complete a picture as possible of the current financial situation for the person under conservatorship as of the date the conservator was appointed. The report is due 90 days after the date of appointment.

 

Large Estate Accounting

The purpose of this report is to give the court as complete a picture as possible of the current financial situation for the person under conservatorship with a large and complex estate. Please read instructions to determine if this is the form you should use.

 

Small Estate Accounting 

The purpose of this report is to give the court as complete a picture as possible of the current financial situation for the person under conservatorship with a small estate. Please read instructions to determine if this is the form you should use. If you complete this form, you do not need to complete the large estate accounting form.
